Confined Space Entry – Underground Utility Vault Inspection
Industry: Oil & Gas · Country: Saudi Arabia · Location: Utility Corridor B – Refinery Complex, Jubail
Risk assessment for entry into underground utility vaults for routine inspection and maintenance of electrical and instrumentation cables. Vaults are classified as permit-required confined spaces due to potential for oxygen deficiency and toxic gas accumulation.
Task Steps
- Obtain confined space entry permit from area authority
- Atmospheric testing for O2, LEL, H2S, and CO
- Set up ventilation equipment and verify adequate airflow
- Assign trained attendant at entry point
- Entrant dons appropriate PPE including SCBA if required
- Conduct inspection and maintenance tasks
- Communicate continuously with attendant
- Exit vault, close permit, document findings
Risk Assessment (1 risks)
Risk #1 — Chemical (high, score 10)
Hazard: Oxygen deficiency or toxic gas (H2S) accumulation in vault
Consequences: Asphyxiation, poisoning, fatality
Control measures:
- Continuous atmospheric monitoring throughout entry
- Mechanical ventilation prior to and during entry
- SCBA available and trained personnel only
- Emergency retrieval system in place
- Rescue team on standby during entry
