A scaffolding worker fell approximately 8 meters from a working platform on a high-rise residential construction project. The worker was repositioning scaffold boards when he lost his balance. He was not wearing a harness at the time of the fall. Despite immediate emergency response and transfer to hospital, the worker passed away from his injuries 6 hours after the incident.

1. Dedicated fall protection supervisor assigned to all multi-level work zones. 2. Works stop immediately if any worker observed not wearing harness at height – zero exceptions. 3. Daily scaffold inspection now mandatory before work commences with photographic evidence. 4. Anonymous safety reporting hotline introduced to allow workers to raise concerns without fear of retaliation. 5. Full investigation report shared with all site contractors as a safety alert.
