A fall from height incident occurred at a construction site. This case study covers the events, causes, and preventive measures.

What Happened

A construction worker was working on scaffolding at approximately 4 meters height without proper fall protection. The worker lost balance and fell, sustaining serious injuries. The scaffolding had not been inspected by a competent person before use.

Immediate Cause

Worker was not wearing a harness and the scaffolding lacked guardrails.

Root Cause

Inadequate fall protection program, missing scaffolding inspection protocols, and failure to enforce PPE requirements at the site.

Lessons Learned & Recommendations

1. All workers at height must wear full body harnesses with lanyards anchored to secure points. 2. Scaffolding must be inspected by a competent person before each shift. 3. Guardrails and toe boards must be installed on all scaffolding platforms. 4. Conduct daily toolbox talks on fall hazards. 5. Implement a permit-to-work system for all work at height above 1.8 meters.
