Safe Confined Space Entry — Oil & Gas

Standard operating procedure for safely entering and working in confined spaces within oil and gas environments.
Task Steps
- Obtain a confined space entry permit from the permit authority
- Conduct atmospheric testing for oxygen, flammable gases, and toxic substances
- Isolate all energy sources and apply lock-out/tag-out (LOTO)
- Establish continuous atmospheric monitoring throughout the task
- Assign a trained standby person at the entry point
- Brief all entrants on emergency evacuation procedures
- Wear appropriate PPE including SCBA if required
- Maintain continuous communication between entrant and standby
- Cancel permit and evacuate immediately if conditions change
Risk Assessment (4 risks)
Risk #1 — Environmental (high, score 16)
Hazard: Oxygen deficiency
Consequences: Worker fatality from asphyxiation
Control measures:
- Continuous atmospheric monitoring
- Proper ventilation
- SCBA equipment available
Risk #2 — Environmental (high, score 16)
Hazard: Flammable atmosphere
Consequences: Explosion causing multiple fatalities
Control measures:
- Gas testing before entry
- Eliminate ignition sources
- Use intrinsically safe equipment
Risk #3 — Environmental (high, score 12)
Hazard: Toxic gas accumulation
Consequences: Poisoning or long-term health damage
Control measures:
- Continuous gas monitoring
- Proper ventilation
- Evacuation at threshold limits
Risk #4 — Operational (medium, score 8)
Hazard: Engulfment
Consequences: Worker burial or crushing
Control measures:
- Isolate material sources
- Blank and bleed lines
- Standby rescue team
